SER Form

Steps to complete our SER form for the upcoming school year:

• Step 1: Download form –  HorizonCharter-Student Enrollment Interactive SER PDF
  • Click the link above and then choose the download icon in your browser
• Step 2: Right-click on form and open with a PDF viewing application such as Adobe
  • Do NOT open in your browser as you will not be able to save your progress
• Step 3: Once completed, please save the document on your computer in a memorable location
• Step 4: Please email the completed document to SER@Horizoncharter.org
• Step 5: Horizon Charter School will have the completed & filled out document ready for your signature
